Can't Afford a Doctor After an Accident? How Medical Liens Work in California

By Gold Coast Law

One of the most stressful parts of being injured in an accident is wondering how you'll pay for the care you need. Maybe you don't have health insurance. Maybe your deductible is too high. Maybe you can't afford to miss more work to chase down referrals. The good news is that in California, you almost always can get the medical treatment you need, even with no money down, through what's called a medical lien.

What Is a Medical Lien?

A medical lien is a written agreement between you, your attorney, and a treating provider. The provider agrees to give you the care you need now and to wait to be paid out of your eventual settlement or verdict. You sign nothing that puts a debt on your credit. The provider gets paid at the end of the case, directly out of the recovery, before the funds are distributed to you.

It's a tool that exists specifically because the law recognizes that injured people need treatment before insurance companies are willing to pay.

How It Works in Practice

Once you become our client, we connect you with a network of physicians, chiropractors, orthopedists, pain management specialists, imaging centers, and surgeons who routinely treat accident victims on a lien basis. You receive the same quality of care as any cash-paying or insured patient.

You don't get a bill in the mail. You don't get phone calls from collections. The provider tracks the cost of treatment, and at the end of the case those bills are paid from the settlement funds. If for any reason the case does not result in a recovery, the lien terms typically protect you from being personally chased for those balances, but every agreement is different, and we explain yours in detail before you sign.

Bilingual Care Is Available

Many of the providers in our referral network speak Spanish or have Spanish-speaking staff. We understand how isolating it can feel to navigate the medical system in a second language while you're hurting. Whenever possible, we match you with a doctor's office where you can be understood without an interpreter.

Why This Matters for Your Case

Getting prompt, consistent treatment matters for two reasons. First, it helps you heal. Second, it documents your injuries. Insurance companies use any gap in treatment as a reason to argue that you weren't really hurt. Liens make sure that financial stress isn't the reason your medical record has a gap in it.
